Technique of Ovarian Vessel Isolation and Ligation for Oncofertility This educational video will demonstrate the surgical steps utilized by the Northwestern University Oncofertility Surgical Team to harvest ovarian tissue for cryopreservation. This technique has been standardized at our institution. It utilizes conventional laparoscopic instruments with the addition of a 5mm vessel-sealing device. The goal of the procedure is to maintain vascular blood supply to the ovary throughout the procedure until time of removal and eliminate thermal damage to the ovarian cortex through the use of a unique smart bipolar vessel-sealing device. The surgical steps include: 1) Inspection of the pelvic anatomy. 2) Identification of the vascular blood supply of the pelvis and the ovary. 3) Development of the avascular space of graves and the retroperitoneum. 4) Identification of the Ureter. 5) Ligation of the utero-ovarian vascular pedicle. 6) Division of the medial leaf of the broad ligament. 7) Isolation and ligation of the ovarian vascular pedicle. 8) Removal of the ovary via an endoscopic retrieval bag.

An exercise in patience: the year of the healing of my chest following FTM chest reconstruction surgery. Surgeon was Martyn Webster, Glasgow, Scotland (now retired). Right side nipple revision by David Dunaway eleven months later. The technique used is a form of peri-areaolar, with a leaf-shaped incision made to remove the excess skin and glandular tissue. The nipple is retained on a pedicle of tissue, raised to a higher position on the chest, and the incision is closed up around the nipple. The right-hand side developed a seroma (buildup of fluid) in the early weeks, causing internal scar tissue which is still slowly breaking down. Patience...
