Rare Side-Effect of Codeine in Nursing Mothers (Nov. 2007)
FDA is alerting healthcare professionals about a very rare but serious side effect that can affect the babies of nursing mothers who are taking drugs that contain codeine. The problem affects mothers who are "ultra-rapid metabolizers" of codeine. Ultra-rapid metabolizers have a specific genotype that causes them to convert codeine to its active metabolite, morphine, more rapidly and completely than other people. This can result in the mother having unusually high morphine levels in her serum and breast milk, and this can put her nursing infant at risk of morphine overdose. A recent report in the literature described a healthy, 13-day-old breast-feeding baby who died of a morphine overdose. His mother was taking codeine at less than the usual analgesic dose. The prevalence of the ultra-rapid metabolizers of codeine varies for different populations. Among Caucasians, the figure is about 1 to 10 percent. Among African-Americans, about 3 percent are ultra-rapid metabolizers of codeine, and among Asians and Hispanics, about 1 percent. The highest prevalence is among some groups of North Africans, Ethiopians and Saudis, where it can be as high as 28 percent. It is important to note that nursing mothers have used codeine safely for many years. Many women are sent home after having a baby with analgesics such as acetaminophen with codeine to relieve episiotomy pain or abdominal cramping and many of these women are also breast feeding. Despite this widespread use, FDA was able to find only the one case where the baby clearly died as a result of morphine overdose from breast milk. This means that many breast fed babies born to mothers who are ultra-rapid metabolizers and who are taking codeine will not have a problem, but some babies may. FDA has cleared a genetic test that can determine whether someone is a rapid metabolizer of a number of drugs, but there's only limited information on using it for codeine. Which means at this point, the test result alone may not correctly predict whether the mother is an ultra-rapid metabolizer of codeine. In other words, the test is not a substitute for a doctor's judgment. FDA recommends that clinicians who prescribe codeine for a nursing mother do so in the lowest dose for the shortest period of time in order to relieve pain. Clinicians should also educate nursing mothers who may be taking codeine about the signs of morphine overdose in themselves or their infants. Mothers should understand that they don't need to go without pain relief if they're breast feeding, but they should know what to look for if there's a morphine overdose. For the mother herself, the signs include extreme sleepiness and constipation. The mother should also watch for increased sleepiness in her baby, keeping in mind that breastfed babies usually nurse every two to three hours and shouldn't sleep more than four hours at a time. She should also watch for trouble breast feeding, breathing difficulties and limpness. Mothers should also be aware that morphine may remain in the infant's body for up to several days after the last codeine dose. Finally, in order to help the FDA understand and quantify this problem, it's important to report possible cases of morphine overdose in mothers and infants. Confederate authorities were unable to requisition enough reliable troopers in this way, and eventually recruiters turned to a new source of fresh meat for the grinder. The obscure planet of Azeroth was found to harbor a dizzying selection of humans, mutants, and aliens in a state of perpetual superstition and conflict. The most physically imposing of these warrior races were the so-called 'tauren,' an anthropomorphic bovine genotype with super-human stamina, overpowered racial combat abilities, and bizarrely well-developed horticultural skills. While most tauren were satisfied with their agrarian culture and primitive existence, the Confederacy was able to lure away large numbers of young bulls for a life of adventure and violence along the galactic rim. Other tauren referred to these adventurers as 'mad cows,' and they were ostracized from the rest of their kine. The newly created tauren marines didn't care--they got neat-looking armor and big guns to own everyone else with. These beefy new soldiers are now an integral part of the Confederate military, and have managed to horn in on the roles traditionally filled by firebats and other infantry units. It's udder chaos! Sean's Golden Labradour Retriever The Labrador Retriever (also Labrador, Labby or Lab for short), is one of several kinds of retriever, a type of gun dog. The Labrador is considered the most popular breed of dog (by registered ownership) in the world, and is by a large margin the most popular breed by registration in the United States (since 1991)[1] the United Kingdom,[2] Poland, and several other countries.[3] It is also the most popular breed of assistance dog in the United States, Australia, and many other countries, as well as being widely used by police and other official bodies for their detection and working abilities.[4] They are exceptionally affable, gentle, intelligent, energetic and good natured,[3][4][5] making them both excellent companions and working dogs. Although somewhat boisterous if untrained, Labrador Retrievers respond well to praise and positive attention, and are considerably "food and fun" oriented. These dogs are as well loyal and great with little children. They may be used in shows. With training, the Lab is one of the most dependable, obedient and multi-talented breeds in the world. Nose and skin pigmentation Because Lab colouration is controlled by multiple genes, it is possible for recessive genes to emerge some generations later and also there can sometimes be unexpected pigmentation effects to different parts of the body. Pigmentation effects appear in regard to yellow Labs, and sometimes chocolate, and hence the majority of this section covers pigmentation within the yellow Lab. The most common places where pigmentation is visible are the nose, lips, gums, feet,tails, and the rims of the eyes, which may be black, brown, light yellow-brown ("liver", caused by having two genes for chocolate),[11] or several other colours. A Lab can carry genes for a different colour, for example a black Lab can carry recessive chocolate and yellow genes, and a yellow Lab can carry recessive genes for the other two colours. DNA testing can reveal some aspects of these. Less common pigmentations (other than pink) are a fault, not a disqualification, and hence such dogs are still permitted to be shown.[11] The intensity of black pigment on yellow Labs is controlled by a separate gene independent of the fur colouring.[11] Yellow Labs usually have black noses, which may gradually turn pink with age (called "snow nose" or "winter nose"). This is due to a reduction in the enzyme tyrosinase which indirectly controls the production of melanin, a dark colouring. Tyrosinase is temperature dependent—hence light colouration can be seasonal, due to cold weather—and is less produced with increasing age two years old onwards. As a result, the nose colour of most yellow Labs becomes a somewhat pink shade as they grow older.[11] A seven-week-old Dudley Labrador Retriever. The nose and lips are pink or flesh-coloured, the defining aspect of Dudley pigmentation, as compared to the more standard brown or black.A colouration known as "Dudley" is also possible. Dudleys are variously defined as yellow Labs which have no pigmented (pink) noses (LRC), yellow with liver/chocolate pigmentation (AKC), or "flesh coloured" in addition to having the same colour around the rims of the eye, rather than having black or dark brown pigmentation.[11][8] A yellow Lab with brown or chocolate pigmentation, for example, a brown or chocolate nose, is not necessarily a Dudley, though according to the AKC's current standard it would be if it has chocolate rims around the eyes (or more accurately of the genotype eebb). Breed standards for Labradors considers a true Dudley to be a disqualifying feature in a conformation show Lab, such as one with a thoroughly pink nose or one lacking in any pigment along with flesh coloured rims around the eyes. True Dudleys are extremely rare.[11][12] Breeding in order to correct pigmentation often lacks dependability. Because colour is determined by many genes, some of which are recessive, crossbreeding a pigmentation non-standard yellow Lab to a black Lab may not correct the matter or prevent future generations carrying the same recessive genes. For similar reasons, crossbreeding chocolate to yellow labs is also often avoided |
