Episode overview
J.D. sees Janitor talking to Todd. Strangely, he is talking to him in an English accent, asking him to try one of his "Tea Buns" (which are actually scones) he is holding. After Todd leaves J.D. tells Doug how he thinks the Janitor is pretending to be English. However, Doug looks confused, and asks if J.D. is talking about "Klaus the German Guy?", as to which the Janitor approaches them and says "Guten Tag Doug!". J.D. finds out that the janitor is actually known by three separate identities; Klaus the Dimwitted German, Ephram the stutterer, and Nigel the Brit.
Back home, J.D. watches the movie ''The Fugitive'' and realizes that Janitor actually played a part in the film - the transit cop on the train near the end. He confronts Janitor, who denies it. (Neil Flynn, who plays the Janitor, actually had the role of the transit cop in ''The Fugitive''.)
Later, the Janitor shows J.D that his true identity is the one he has been to J.D all along, but then says: 'If you tell anybody, I'll kill you.'
Meanwhile, Carla is worried about the fact it will be a long time before Turk is ready to have children, and Dr. Cox, scared by the idea of growing old, attempts a risky basketball move, injuring his back in the process.
Cultural references
★ References to Shaft are made including during a day dream after Turk's successful surgery and following a success of JD's.
★ JD uses the phrase "I put my pants on one leg at a time just like you guys. Except once my pants are on I save your husband's life." This is a reference to the well known SNL skit parodying Blue Öyster Cult's hit "(Don't Fear) The Reaper" and its use of the cowbell. The line is delivered by Christopher Walken in the skit, except his is finished with "...once my pants are on, I make gold records."
★ The car crash scene from the beginning is taken from the movie Vanilla Sky.
Featured music
★ "Stuck in a Moment You Can't Get Out Of" by U2
★ "Hero" by Enrique Iglesias
